Health and Purchase
Agreement

I, Linda Norton~Smith have received $_______for a deposit on ___ poodle puppy. This deposit is NON-
REFUNDABLE
. The deposit is to hold said puppy until it is 8 weeks of age and ready for new home placement. If buyer
fails to proceed with full purchase price of puppy for ANY reason, seller will keep deposit for loss of sale of pup.
Your puppy should be ready for new home placement on ______________. I recommend taking the new pup home on a
weekend, especially if the buyer works throughout the week. This gives the pup a chance to get use to his new owners
and new surroundings, without starting out at home alone. I would hope that you have given extreme thought to the extra
care and attention that a new puppy requires. It is a full time responsibility to make sure that the puppy gets plenty of
food and water in order to remain a healthy thriving puppy. I would hope that your new pup will bring you many years of
undying love, happiness, companionship and loyalty and I pray that you would offer him/her the same. Our babies are
loved from birth and we would like to know they are always treated with sincere love and compassion. This contract was
placed in effect on ______________.


We make every effort to guarantee that each puppy from Southern Lady's Red Poodles is in good health at the time of
adoption/delivery. Therefore, we ask that you have it
checked by your vet within 48 hours of acquiring your puppy.
We cannot control the exposure your puppy encounters after leaving us. If you choose not to have your puppy
examined,
our contract is VOID. We warrant the health of your puppy the first 12 months of life against these life-
threatening illnesses (heart, liver, & kidneys). Puppies sometimes stress when adopted and moved to their new home.
This stress can be brought on due to the move, change of diet, too much handling, not enough quiet time, being taken
away from siblings / mom, or not eating properly. Stress can also bring on things that are dormant, such as, but not
limited to worms, ear mites, colds, giardia, hypoglycemia and/or coccidiosis. These conditions are treatable & DO NOT
warrant a return. Should your puppy have a life-threating condition, it can be replaced with another puppy of equal
price, size, and / or color when available. However, these guidelines  must be followed in order for a replacement to be
made.
No cash refunds will be made.
1.        The new puppy MUST BE RETURNED along with all the paper work. Immunizations must be in order and up to
date. No exchange will be made unless this is done.
2.        If the puppy dies, an autopsy MUST BE DONE by licensed veterinarian stating the cause of death. The
documentation must be forwarded to the breeder before a determination will be made. All expenses that are incurred will
be at the BUYER’S EXPENSE. This includes returning the puppy to the breeder, and any vet charges incurred whether
for autopsy or to diagnose a problem. Our contract is null and void in the case of accidental injury. Small puppies are
very fragile and easily injured . Accidents do happen and every precaution should be taken when handling them. Many
questions are asked regarding the size, personality or color of a puppy. We can only estimate size based on prior litters
and / or size of the parents. We make no guarantees on size, color, fertility, temperament.
